Second Opinions

Dr. Kulisz provides second-opinions to patients, medical doctors, naturopathic doctors, chiropractors, and other natural health practitioners. To issue a second opinion, we need the following information:

Patient’s name
Contact information: email, complete landline telephone number with country code (if outside of the US and Canada) and area code, Skype ID if available.
Patient age, gender, and race
Doctor’s/practitioner’s name
History of general health
History of health problem, applied treatments, results. Provide documented tests, opinions, charts, etc., if available for the last six months to a year.

Expect two in-person or telephone/Skype interviews for about 30 – 45 min. each.

Time and cost: Estimated total billable time — 5 hours. This includes a review of information, interviews, and opinion. If a written opinion is not needed — one hour less. Billable rate is $200/hr plus expenses (mail, couriers, travel if necessary, etc. Opinions rendered in American English. If translation into another language is required, the original American English version prevails. We do NOT translate or arrange for translations of our opinions. Dr. Kulisz is not responsible for the accuracy of translations and possible, resulting errors in treatments.

All opinions must be prepaid in full. Additional expenses will be billed to the credit card on file (if we have it) or by PayPal within 48 hours after delivering the opinion.

Mode of payment: PayPal (to the email address drakulisz@kulisz.com), Visa, MasterCard, Discover, Diners Club, wire transfer, personal check, cash. Payment must clear at least three days before consultation.

Persons paying by credit cards, please send email to MaryWhite@kulisz.com from YOUR email address with the following statement:

I (your first and last name) authorize Dr. Kulisz to charge my (name of the card) credit card ending with (last four digits of the card number) for the amount of $(800 or 1000) plus expenses not to exceed $200 if required.
